How It Works

About Ozempic®

Ozempic® is a once weekly injectable medication that contains semaglutide, a GLP-1 receptor agonist. It works by activating the same receptors as the body’s natural GLP-1 hormone. This helps regulate blood sugar and slows how quickly food moves through the stomach. Together, these effects help reduce appetite, support more stable eating patterns, and reduce hunger spikes.

Ozempic® comes in several once-weekly dose strengths. Most people start with:

  • 0.25 mg once weekly for the first 4 weeks, which helps the body adjust
  • 0.5 mg once weekly after dose escalation

If more support is needed, your provider may increase the dose to 1 mg once weekly.

Dose increases are based on how you feel, how well you tolerate the medication, and your overall response.

Ozempic® is injected under the skin once a week, on the same day each week. It can be taken with or without food.

  • Inject into the abdomen, thigh, or upper arm
  • Rotate injection sites and avoid using the same spot repeatedly
  • Never inject into a muscle or vein
  • Inspect the solution before use; it should be clear and colorless

Missed Dose

If you miss a dose, you can take it within 5 days. After 5 days, skip the missed dose and take your next dose on your usual day.

If you use insulin, the two injections must be given separately and never mixed.

The most common side effects involve the stomach and digestive system. These often occur during dose increases and usually improve as your body adjusts.

Common effects include:

  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation
  • Abdominal pain or discomfort
  • Indigestion
  • Decreased appetite
  • Fatigue or dizziness

Less common but more serious reactions can involve pancreatitis, gallbladder issues, kidney problems related to dehydration, worsening of diabetic retinopathy in people who already have it, and rare allergic reactions. Anyone experiencing troublesome or persistent side effects should seek medical attention.

Ozempic® delays gastric emptying, which can affect how some oral medications are absorbed. It can also interact with certain drugs that influence blood sugar, heart rate, or heart rhythm.

Caution is advised with:

  • Drugs that increase heart rate
  • Medications that prolong the PR interval, including certain antiarrhythmics, beta blockers, calcium channel blockers, digitalis glycosides, and some HIV medications
  • Warfarin and other coumarin based anticoagulants, since your INR (the time it takes your blood to clot) may change when you begin taking Ozempic®.
